Supporting Disability Documentation Requirements
Disability verification documentation must include in order to qualify for academic accommodations:
- Either be in English or have attached to the original documentation a certified English translation;
- Confirm the rationale for seeking accommodation;
- Be from a credentialed health professional;
- Include the credentialed health professional’s name, title, and contact information;
- It must be stamped with the professional’s stamp or be on official letterhead;
- Include the date of the assessment;
- Include a statement on the nature of the disability and the care plan including any medications and the impact of not following the care plan;
- Explain the impact of the disability on the student in a post-secondary learning environment; a diagnosis alone is insufficient.
If you need Schedule IV Disability Funding, it is recommended that you have a qualified healthcare professional complete the Disability Verification Form below. Even if you are not applying for this funding, we will accept this completed form as supporting documentation.
